Decision Debt: The Operational Liability You're Not Tracking

Can I ask you the question I have been asking myself?
When your team asks what you decided last time, does someone have to stop and rebuild the answer from three months of chat?
You know that moment. The meeting loops. The same call comes back. A new hire takes twice as long because the why still lives in one head. Keeping it there felt like being the person who still knows. Collecting one more data point felt like care. What it actually was is finished work, coming back for another full pass.
The cost is not the original call. The cost is remaking it every time the question resurfaces. Hours go into the search. Real work waits. You stay in the loop because the answer has nowhere else to live.
The tells are ordinary:
The Search
Someone asks who owns this. Three people reconstruct it from Slack.
The Loop
The weekly meeting covers a call you thought you closed last month.
The Remake
Budget, scope, and exceptions keep requiring you again.
The Fix: Capture the decisions that keep coming back
This is not a binder. Write the call where people already look. If that is the Slack channel for the work, put the owner and the why there. If the team already lives in a Notion page, use that page. Keep it to a few lines. Your job this week is not to document everything. It is to stop remaking the same few decisions from memory.
